Treatment informed consent generator
A clinical template for Botox, filler, laser, physiotherapy, psychotherapy and more — logo, procedure clauses and space to sign.
Data consent does not cover the clinical act
A GDPR consent form authorises processing of health data. Treatment informed consent is a different document: it explains the nature, risks, alternatives and aftercare of a specific procedure, and records that the patient decided with enough information.
This tool builds a guidance template by specialty and procedure (botulinum toxin, filler, laser, peel, physiotherapy, infiltration, psychotherapy and others). Fill in clinic details, the name of the act and, if you wish, your logo. The PDF is created in the browser, with no account.
It does not replace clinical judgement or legal advice. Adapt it to the case and review it before use. In Spain it draws on Law 41/2002; in Portugal, on Law No. 15/2014.
Legal and clinical notice
This template is for guidance only and is not medical or legal advice. Review it for the procedure, the patient and the rules of your region or country before using it in clinic.

In Domeralia the document fills with the patient record
This template is a starting point. In Domeralia, PDF document models are generated from the patient file, ready to sign or send, with the same format across the clinic. GDPR data consent is a separate document — you can generate that here too.
