In Google Business Profile, find the option to ask for reviews or copy your Google Maps listing link. That URL belongs in the QR: when scanned, the patient goes straight to writing a review, not to a generic search.
Print the poster on A4, laminate it if you can, and place it at eye level at reception or in the waiting room. A tiny QR in a corner does not get used.

Is the QR code generated on your servers?
No. It is drawn in your browser from the link you paste. We do not send the URL or the logo to Domeralia.
Can I use a Google Maps link instead of the review link?
Yes, the QR will work. The dedicated review link usually saves the patient a tap. If you only have the Maps listing, use it and update the poster when you have the direct link.
